What to expect on
Your First Visit
Your initial appointment will at minimum consist of a consult with a Dentist explaining your diagnosis and treatment options. At this first visit, it is preferable that a cleaning be conducted by one of our skilled hygienists. It is important that you arrive at least 15 minutes prior to your first appointment to complete all the necessary questions and information required on your new Dental Chart.
Required Information
Please assist us by providing the following information at the time of your consultation
- A list of medications you are presently taking.
- If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
A dental cleaning and examination can be carried out at the first visit, however; a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at each visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If your dentist has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.

Before coming in for your first visit, you can download our patient forms from the link below. Feel free to print out these forms and bring them in with you on your first visit! Our office requires 2 business days to change or cancel any appointments to avoid charges.
